This page of the online order form (Step One) asks you to write the id numbers of the ladies you want to contact. It will also calculate for you the cost for the addresses you wish to order. This information is not confidential so this first page is not yet at the secure server. Clicking the submit button at the bottom of the page will lead you to the secure server (Step Two).

Type in the id numbers of the ladies whose addresses you wish to order. Each id number is 5 digits long. If you  use "Your Favorites", you can stuff the form with the id numbers of the ladies from "Your Favorites" by clicking on  "YOUR FAVORITES" automatically by clicking here.  If you have saved more "Your Favorites id numbers than you wish to order at this time, just delete the extra id numbers from the page.  It is ok to leave blank spaces in the id number fields.

After your list of desired id numbers is ready then calculate the cost by clicking on  "CLICK HERE to calculate cost".

The next field asks  "Please enter an Alternate Member Numbers, in case a member # you're ordering has expired.)" This field is optional and your order will go through with it blank. If you have identified an extra lady whose address would be suitable in case one of your primary choices has become unavailable while your order is being processed, it will speed up your order if you provide the alternate id number.
 
The next two fields ask if you want to subscribe to the First Choice Alert mailing lists. This is optional. Only fill in these fields if you wish to subscribe.

Finally, at the bottom of the page you will see a button labeled "Proceed to Next Step".  Click on this button to proceed to our secure server.
